Perfect for Patient Centered Medical Home (PCMH) and Accountable Care Organization (ACO) initiatives, Axchange Care allows healthcare provider organizations to harness patient data to engage patients and drive population health outcomes.
Use Axchange Care to connect Care Managers with patients while keeping physicians in the loop. Integrate clinical records, claims history and patient-collected data to inform phone-based patient counseling. Power outreach to engage patients, and produce clinical quality reports that substantiate the value of patient-centered and Accountable Care models.
Axchange Mail
Axchange mail is a directed exchange solution based on the NwHIN Direct specification and following an email paradigm that may be accessed with many popular email clients. Our functionality provides the ability to securely pass protected health information between trusted sources.
The included Provider Directory services make finding authorized healthcare providers easy and allows users to quickly address secure messages to those listed. Alphajen follows the Healthcare Provider Directory (HPD) IHE specification.
Axchange Data Match
Data Match is engineered to be an enterprise class Entity Identification Service built on open standards and leveraging the strengths of existing open source components to provide a powerful, scalable and comprehensive solution capable of handling common healthcare Master Data Management (MDM) patterns such as Enterprise Master Patient Index (EMPI), Master Provider Index, Facility index or simple member list deduplication.
Built as a reference implementation of the HSSP Entity Identification Service (EIS) specification, Data Match correlates or cross references identifiers from Patients or Providers (Entities) located within disparate systems or sources. The standard EIS webservice interfaces provide the common thread by which entity data can be indexed. The unique identifier and standard way to search, retrieve and manage entity data will allow healthcare applications and healthcare enterprises to find, exchange and reference entity data while maintaining the data’s context and associations.
Benefits Maintain a single consistent global identifier linking local identifiers from many disparate sources
Detect and or avoid Entity duplication thereby improving data quality
De-duplicate existing entity stores
Axchange Results
Results can collect, organize, and aggregate clinical data from many different sources. This clinical data is easily viewed and queried, and patient matching across these sources occurs seamlessly coupled with Data Match. The types of clinical data that can be stored in Results currently include full patient demographics and clinical observations such as laboratory and imaging results.
All accesses to clinical data are fully audited, and a user’s ability to view this data can be restricted to specific providers, source data streams, and/or delivery sites. Incoming data streams can be further transformed, filtered, and delivered to participating EMRs/EHRs to keep them synchronized with the CDR. Results supports flexible deployment patterns ranging from a single centralized instance to a more distributed, compartmentalized topology.

Axchange Benefits
Alphajen’s Axchange provides a healthcare platform that allows for integration of clinical data for use at the point of care and to aid in clinical decision-making, required for meaningful use of EHR technology.
Axchange provides for interoperability for applications utilized in the physician practice environment including the electronic health record (EHR), the personal health record (PHR), computerized physician order entry (CPOE), clinical decision support (CDS), picture archiving and communications systems (PACS), and e-prescribing. Allowing healthcare entities to implement and execute cost-containment strategies (for example, cost-effective revenue cycle processes, more efficient management of patients within systems, and a reduction of the laborious accreditation and credentialing steps).
Provides a wealth of information at the time and place of care
Provides reliable and accurate patient history
Enables providers to perform a more accurate, focused patient evaluations
Reduced costs to patients and healthcare system
Improved overall patient care
Benefits to Hospitals
Hospitals are able to collaborate with and assist physicians to prevent readmissions and better manage patients. Physicians generally are eager to improve efficiency because it frees them to see more patients, with the support executive leadership to ensure care coordination efforts succeed.
Focus on the cost per episode and more broadly on the cost per capita and the broader cost across the entire continuum for the entire population you serve.
Coordinating care to reduce outpatient-sensitive admissions and hospital readmissions, and providing preventive and wellness care to keep people out of the hospital.
Allow for management and administration of bundled and capitated payment programs, and to focus on cost per episode and cost per capita.
Cost Reduction:
Standardizing care
Removing unnecessary care, including reducing provider errors, preventable readmissions, avoidable conditions and unnecessary Diagnostic tests
Cost restructuring to use the lowest cost setting and provider possible for each service
develop a system of care strategy, to include strategies such as medical homes and disease management to reduce overall hospital service demand. Must redesign care focusing on the disease level, and engage physicians and clinic partners inside and outside the hospital, and continuously measure and improve performance
Benefits to ACO's
The Axchange provides the foundation for any ACO-Care system, and facilitates numerous transactional services, that deliver information to agencies as APIs or “web services” allowing for the uninterrupted flow of data throughout the Axchange on behalf of the ACO.
SAAS-based system available to every physician within the ACO, allows a practice to poll the Axchange and find a patient’s medical record across the entire organization. Typically the medical record is restricted to labs, medications, vitals, immunizations, allergies, past/family/social history and problems. Sharing of chart notes requires prior permission form the physician.
The Axchange allows for the integration of physicians with disparate EHR systems as well. All certified EHR systems must be able to send and receive CCR (Continuity of Care Records) in order for a rapid and efficient exchange of information. By utilizing the native CCR functionality of EHR systems, a physician organization comprised of dozens of varying EHR systems can be integrated to the Axchange quickly.
Connectors to all other EHR systems
Axchange provides a seamless connection to all practices in the ACO by importing data from all major EHR systems into the Axchange platform.
Data may be imported in the following formats:
XML
HL-7
CCR
CCD
Connectors to all hospitals
Through the use of Axchange import and export technologies, hospital interfaces into and out of the Axchange can be quickly implemented for most hospital systems. ED personnel to quickly look up patient data from the Axchange. ED personnel can obtain all medications on file from all US pharmacies participating with RXHub/Surescripts via the Axchange.
The physician-hospital connectivity is critical for physicians to receive discharge summaries and hospital lab data. The hospitals also benefit by having access to patient medications and lab results from the surrounding physician community, especially within the Emergency Department (ED).
Connectors to all laboratories
Alphajen specializes in developing connections to numerous laboratories and LIS, including reference labs, lab information systems (LIS) such as Orchard, LabDaq, Fletcher-Flora and others. Alphajen can create an interface if the lab can import and export HL7 data.
This allows the ACO system to integrate all labs and LIS products easily and effortlessly and create a solid patient record in the
Axchange that can be shared by all physicians under the ACO.
Connectors to all radiology practices
Alphajen can develop interfaces with numerous radiology and MRI units to create PACS links and allow providers to view radiology and MRI images easily from within the ACO. Alphajen will not alter or modify the images, and will make them available through interface devices so physicians can securely view radiology results for their patients. Radiology reports are stored securely in the Axchange for to maximize data sharing capabilities.
